Overview of Forms and Reference Materials

The following is a list of forms and reference materials that are available for use by managers and staff members during the Performance Partnership process. The forms are Microsoft Word documents that can be downloaded from the HR web site. Save the form to your computer and them complete the form by navigating to each section of the forced form with the Tab key. The gray areas will expand as you enter information. Please contact HR if you experience any difficulty in completing these forms.

Tools Available Description
Self-Assessment form Staff members should begin the Performance Partnership process by completing this form and submitting it to their supervisor prior to the formal performance feedback meeting.
Performance Review Forms:
General form
Narrative form
PLEASE NOTE: Category specific forms have been condensed into one General form. The new Narrative form is optional but recommended for staff in job categories S6 and above.
Goals/Development Worksheet This worksheet captures specific staff member goals (i.e., expectations, development activities, projects, etc.) and monitors progress throughout the next review period.
Competency Library Competencies (i.e., skills, knowledge and behavioral practices) that are related to the work environment are listed in the Competency Library. Each Competency also includes behavioral indicators that describe what one would "see" if the competency was displayed.
Pay Administration Handbook This resource provides examples and descriptions of practices used to link pay to performance.
