Purpose: To document the accomplishments, overall performance and goals and development opportunities for the next review period for staff members in S6 level positions and higher.

The Narrative form was developed to respond to an expressed need for flexibility in preparing performance reviews for higher level positions at the University. The narrative format assumes the mastery of advanced level competencies for these positions and, therefore, focuses only on the evaluation of goal achievement, development opportunities and goal setting for the next review period. A supervisor reviewing someone at these higher levels may choose to use either the General form or the Narrative form.
