Overview of Syracuse University Medical Aid Program (SUMAP)
SU is sensitive to the financial burden some faculty and staff face due to the high cost of medical care. For this reason, SU introduced in 2009 the Syracuse University Medical Aid Program (SUMAP) to provide some relief for faculty and staff with the greatest need.
Applications for SUMAP in 2010, together with the requested financial documentation, are due 12/1/09. Employees must apply each year for benefits effective the following calendar year. HR will review late applications and place those that qualify for the program on a waiting list for assistance in 2010.
Eligibility for this program is based on household taxable income and household size (number of dependents), relative to the 2009 Federal Poverty Level.
Faculty and staff must complete an application process and those who qualify for SUMAP will receive assistance in one of two ways:
- Group 1 Benefits - Qualified faculty and staff are expected to receive a 75% reduction in their SU medical plan out-of-pocket maximum and a 25% subsidy to their monthly plan contributions. The out-of-pocket (OOP) maximum is the annual amount a member has to pay out of his/her own pocket for medical plan copays, deductibles, and coinsurances. Monthly plan contributions are not included in the out-of-pocket calculation. Faculty and staff whose taxable household income and size place them at 0-150% of the Federal Poverty Level will receive Group 1 benefits.
- Group 2 Benefits - Qualified faculty and staff are expected to receive a 75% reduction in their SU medical plan out-of-pocket maximum.
Employees whose household income and size place them at 151-250% of the Federal Poverty Level will receive Group 2 Benefits.

Applying for the Program
Employees must apply each year for benefits effective the following calendar year. The application process involves completing a form and providing additional financial documentation. The financial criteria includes submitting a copy of the federal tax return from 2008 and W-2 forms for you and your spouse/same-sex domestic partner from 2008. (For example, to apply for benefits effective in 2010 provide a copy of your 2008 federal tax return.)
Exceptions may be made if an employee can substantiate a significant change in household income or size since the most recent tax return.
